Prolactin and Fertility: What You Need to Know

Prolactin is a hormone produced by the pituitary gland. It plays a key role in breast development and milk production after childbirth. However, high levels of prolactin (hyperprolactinemia) outside of pregnancy or breastfeeding can interfere with reproductive health in both women and men.

At Fertilite, we help patients identify and manage prolactin-related problems to improve their chances of conceiving.

Why Is Prolactin Important?

In women, prolactin helps regulate the menstrual cycle and prepares the breasts for lactation.

In men, prolactin has a minor role but very high levels can affect testosterone production and sperm quality.

When prolactin is too high, it can suppress other reproductive hormones (LH and FSH), causing menstrual irregularities, absence of ovulation, reduced libido, or even infertility.

Common Causes of High Prolactin (Hyperprolactinemia)

  • Prolactin-secreting pituitary tumors (prolactinomas)
  • Certain medications (antidepressants, antipsychotics, blood pressure drugs)
  • Hypothyroidism
  • Stress or significant physical exertion
  • Chest wall injuries or surgery
  • Identifying the underlying cause is essential to determine the right treatment.

Symptoms of Hyperprolactinemia

Women may experience:

  • Irregular or absent periods
  • Difficulty getting pregnant
  • Galactorrhea (milk-like discharge from the breasts)

Men may experience:

  • Decreased libido
  • Erectile dysfunction
  • Infertility

How We Diagnose It

At Fertilite we perform:

  • Blood tests to measure prolactin levels (usually taken in the morning, fasting)
  • Evaluation of thyroid function
  • MRI of the pituitary gland if needed to rule out adenomas
  • Review of medications and medical history

Treatment Options

Treatment depends on the cause and the patient’s reproductive goals:

Medication: Dopamine agonists (such as cabergoline or bromocriptine) can effectively lower prolactin levels and restore ovulation or improve sperm quality.

Adjusting other medications: If drugs are causing high prolactin, alternatives may be considered.

Treating underlying conditions: For example, correcting hypothyroidism.

Surgery: In rare cases, large prolactinomas may require surgical removal.

Most patients respond well to medical therapy and see an improvement in their fertility.
