When we face challenges in achieving pregnancy, it is important to gather updated information about the reasons behind it and to explore the best possible solutions without delay. Infertility is defined as the inability of a couple to conceive after a year of regular unprotected sexual intercourse.

The World Health Organization (WHO) acknowledges this condition and emphasizes that if a woman is over 35 years old and has been unable to conceive after 6 months, it is advisable to seek the expertise of specialists in Fertility and Reproductive Medicine.

Step By Step

Here are some of the diagnostic procedures that you’ll complete:

1. A physical exam, significant medical history, and transvaginal ultrasound.

2. Laboratory tests such as AMH (Anti-Müllerian Hormone), hormonal profile (FSH and LH), Estradiol, and Prolactin, among others.

3. If necessary, complementary studies such as Hysterosalpingography or Hysterosonography will be performed.

4. Semen analysis or direct Spermatobioscopy (DBE), to check the quality of the sperm.

Consulting And Planning

Once all the test results are available, a diagnostic integration meeting will be held. In this conversation are presented possible therapies that are more suitable for each couple. They are individualized and the couple must understand the reason for each step.

Possible Therapies

For the American Society for Reproductive Medicine (ASRM) the age of the woman has a direct relationship with the quality of the egg and is coupled with factors such as endometriosis, fallopian tube damage, low semen quality; in vitro fertilization (IVF) treatment alone or in combination with ICSI, will offer the best chance of achieving a pregnancy.

IVF – In Vitro Fertilization

For the past 40 years, in vitro fertilization (IVF) has been a successful method in helping couples conceive. More than eight million children worldwide have been born through this technique, with the number increasing by half a million every year. IVF involves the fertilization of eggs by sperm in a laboratory setting, allowing for the initiation of embryonic development. These embryos are then transferred into the uterus, giving couples the opportunity to start a family.

Stages of In Vitro Fertilization

Ovarian Stimulation

Ovarian stimulation involves the administration of hormones during the treatment cycle. The developing follicles are monitored through ultrasound. This process is tailored to each individual patient and adjusted based on their clinical response. The aim is to achieve mature follicles that indicate a higher chance of successful ovulation.

Egg Retrieval

Once the ultrasound confirms that the follicles have reached the appropriate size and number, they are retrieved through an outpatient procedure called ovarian puncture. This is performed in the operating room under sedation, with a short recovery time and minimal discomfort. Patients can resume their normal activities immediately after the procedure.

Fertilization of the Eggs

After the retrieval, the obtained eggs are assessed and then fertilized with either the partner’s or a donor’s sperm, which has undergone prior preparation. There are two methods of fertilization: Conventional IVF, where an egg is combined with a sperm sample on a culture plate; and ICSI, where a single sperm is injected into the egg using a microscopic puncture.

Embryo Transfer

Developing embryos are closely assessed with the best-quality embryos selected for transfer. This painless outpatient procedure is performed with ultrasound. To determine the outcome of the transfer, a blood test is conducted to measure the beta fraction, confirming the pregnancy. Subsequent ultrasounds will be performed to monitor the progress.

Embryo Vitrification

Any remaining high-quality embryos that were not transferred can be cryopreserved using a technique called vitrification. This allows them to be stored in the laboratory for future use, ensuring their preservation and viability.
