Step By Step

Our experience has allowed us to design a personalized fertility program for each patient according to their life stage. This often involves specific tests, gynecological procedures, and fertility techniques.

The following are some of the examinations that will be conducted to establish a diagnosis:

Physical examination, review of important medical history, and transvaginal ultrasound.

Laboratory tests such as: Anti-Müllerian Hormone (AMH), hormonal profile (FSH and LH), Estradiol, and Prolactin, among others.

Additional studies will be performed to assess the fallopian tubes and uterine cavity, such as Hysterosalpingography, and in some cases, Hysterosonography or Hysteroscopy.

Counseling and Planning

Once all test results are available, a meeting will be held to discuss the diagnosis and present the ideal treatment protocol for the patient.

Possible Therapies

Case 1

Intrauterine Insemination (IUI) with donor sperm from a sperm bank, previously chosen by the prospective mother. This procedure is recommended when there is good ovarian reserve, a healthy uterus, and open fallopian tubes.

Case 2

In Vitro Fertilization (IVF) with donor sperm, indicated when intrauterine insemination attempts have failed or there are uterine lesions and low ovarian reserve.

Case 3

IVF with Egg Donation and donor sperm. This option is indicated when the patient has a low ovarian reserve or has experienced failures in previous fertility treatments. This option improves pregnancy rates.

Artificial Insemination – Intrauterine

Intrauterine artificial insemination is a low-complexity assisted reproductive technique that optimizes the arrival of sperm to the fallopian tube for fertilization. This increases the possibility of pregnancy.

Who is it recommended for?

Intrauterine Artificial Insemination is indicated for women up to 38 years old with acceptable ovarian reserve, open fallopian tubes, or at least one quality sperm with minor to moderate abnormalities in the male partner. The time spent trying to conceive should be less than 3 years.

The number of cycles may vary. For those under 35, if pregnancy isn’t achieved after three attempts, consideration should be given to moving to In Vitro Fertilization (IVF).

Stages of Intrauterine Artificial Insemination

Ovarian stimulation

Hormones (gonadotropins) are administered for 7 to 10 days to achieve appropriate follicle size, maturity, and ovulation of the oocyte.

This process is personalized for each patient.

Semen Preparation or Sperm Capacitation

The frozen sperm sample is processed in the laboratory according to standard protocols, including thawing, washing, selection, and capacitation.

The laboratory provides the clinically prepared sample (sperm with improved mobility and fertilization capacity) just before insemination.

Insemination

Insemination is a simple, quick, and painless technique performed on an outpatient basis.

It involves placing the sperm sample into the uterus using a catheter. After the sample is deposited, the catheter is removed. The patient can typically resume normal activities in about 20 minutes.

Pregnancy test

After 15 days and using a blood sample, pregnancy hormone levels (beta hCG) are measured. The pregnancy stage is determined based on the levels.

In Vitro Fertilization (IVF)

In vitro fertilization (IVF) has been successfully used for 40 years. Over 10 million children have been born worldwide using this method, with half a million more born every year. IVF involves the fertilization of oocytes by sperm in the laboratory, followed by transferring the resulting embryos into the uterine cavity.

Why IVF Might Be Recommended for a Young Woman Without a Partner

Infertility

When attempting to conceive for an extended period without success, IVF might be recommended after exhausting less invasive treatment options.

Genetic Abnormalities

If there’s a hereditary abnormality, IVF can involve preimplantation genetic testing to select embryos without those abnormalities.

Fallopian Tube Issues

If the fallopian tubes are blocked or damaged, IVF can be a viable option for conception since fertilization occurs outside the body.

Unknown or Multiple Causes

In some cases, multiple factors contribute to infertility without a specific cause identified. IVF might be an option in such cases.

Severe Endometriosis

In cases of advanced endometriosis that affects ovarian function or causes significant adhesions, IVF may be considered.

Diminished Ovarian Reserve

If Anti-Müllerian Hormone levels or follicle count are low, IVF could help maximize pregnancy chances by using multiple eggs in a cycle.

Previous Treatment Failures

If other fertility treatments, like artificial insemination, have failed after at least three attempts, IVF might be the next option.

Previous Surgeries

If a woman has had surgeries affecting pelvic or reproductive anatomy, IVF might be a more effective option for achieving pregnancy.

Steps Involved in an In Vitro Fertilization (IVF)

Ovarian stimulation

This involves administering FSH or a combination of FSH and LH hormones during a treatment cycle, followed by ultrasounds to assess ovarian response. Ovarian stimulation is tailored to each patient, aiming to obtain mature follicles that indicate good ovarian reserve.

Ovarian Puncture

Once the follicles reach the appropriate size and number based on ultrasounds, oocytes are retrieved through ovarian puncture. This outpatient procedure is performed in an operating room under sedation. It’s brief and usually symptom-free, allowing the patient to resume daily activities immediately.

Fertilization of Oocytes

After obtaining and evaluating the oocytes, they are fertilized with sperm from the partner or a donor, previously prepared. Fertilization can occur in two ways:

Conventional IVF: An oocyte is placed in a culture dish with sperm.

ICSI: An individually selected sperm is injected directly into the oocyte using a microscopic needle.

Embryo Transfer

Once suitable embryo development is observed, high-quality embryos are selected for transfer. This painless outpatient procedure is done under ultrasound guidance. Embryo transfer typically occurs 5 days into development, allowing a maximum of 2 embryos to be transferred to the uterus. A blood test measures beta-hCG to confirm pregnancy and subsequent ultrasounds verify embryo vitality.

Embryo Vitrification

High-quality embryos from days 5 to 7 of development that aren’t transferred are cryopreserved using vitrification for future use. These embryos are stored in the embryology laboratory until needed.
